By signing below, I acknowledge and agree that:

1. Taekwondo and related physical training involve physical exertion and inherent risks. These may include slips or falls, contact with other people or equipment, kicks or strikes, sprains, strains, fractures, concussion, and other injuries.
2. I voluntarily choose to participate, or allow the student named above to participate, in a Prime Taekwondo Trial Class. I will tell Prime Taekwondo staff about relevant injuries, medical conditions, allergies, or physical limitations before participation, and the participant will follow instructor directions and safety rules.
3. I understand that no physical activity can be made completely risk-free. I knowingly accept the ordinary and inherent risks associated with participation in a Trial Class.
4. To the fullest extent permitted by applicable law, I release and hold harmless Prime Taekwondo, its owners, instructors, employees, volunteers, and facility operators from claims arising from the ordinary and inherent risks of participation. This agreement does not waive any right or claim that cannot legally be waived.
5. If urgent care is reasonably necessary and I cannot be reached, I authorize Prime Taekwondo staff to arrange reasonable first aid or emergency medical assistance. I understand that I am responsible for costs charged to me by medical providers, subject to applicable law.
6. I confirm that I am the participant if I am 18 years of age or older, or that I am the participant's parent or legal guardian and have authority to sign. I have read and understood this waiver. My typed name and required acknowledgements below are my electronic signature.
